If the recipient of whom you are filling out this form for has never received a bear from us, this is the form you need to fill out.
We can only send one bear per family (bear comes with a shirt already). If you (or the family you are requesting for) has already received a bear and you are wanting another shirt for the same loss or for a separate loss, you need to fill out a shirt request form here.
If you have never received a bear, and would like more than one, there is an option to order additional shirts on the request form below.
